Diagnosis Coding
ICD-10-CM links the ulcer to diabetes with a combination code for diabetes with foot ulcer, paired with a code for the ulcer's site and severity. The severity in the ulcer code (limited to skin breakdown, fat layer exposed, muscle involvement, bone involvement and others) should agree with the depth documented in the note and the debridement code billed.
Debridement
Debridement is selected by depth of tissue removed: 97597 and 97598 for selective debridement, and 11042 to 11047 for surgical debridement to subcutaneous tissue, muscle or fascia, or bone. Units are based on surface area in 20 square centimeter increments.
Offloading And Standard Care
Offloading is the core of diabetic foot ulcer treatment and a required element of standard care before skin substitutes. Document the device or method, the dates, and the patient's adherence. Total contact casting has its own CPT code, 29445, and is subject to bundling with other procedures on the same foot.
Skin Substitutes
When an ulcer fails to respond to documented standard care, skin substitute application on the foot is reported with 15275 to 15278, plus the product code. Coverage, product lists and application limits come from the MAC's policy for the location of service.
E/M Visits On Procedure Days
Debridement codes include the routine pre-procedure assessment. An E/M visit on the same day needs a separately identifiable problem, such as a new infection, a new complaint or a significant change in management, documented apart from the procedure and reported with modifier 25.
Frequently Asked Questions
- Can I bill debridement every week?
- Weekly debridement can be appropriate when the wound bed requires it and the note documents tissue removed and progress. Frequency without documented need is a common denial reason.
- Do diabetic patients need therapeutic shoes documented separately?
- Yes. Therapeutic shoes and inserts follow a separate DME MAC policy with their own certification requirements.
