Billing Compression Therapy And Multilayer Wraps

Compression is the standard of care for venous leg ulcers and a prerequisite for most advanced wound therapy. Whether the application is separately payable depends on what else was done to the same leg at the same visit.

The Application Codes

  • 29580: Unna boot application
  • 29581: multilayer compression system, leg below the knee, including ankle and foot
  • 29584: multilayer compression system, upper arm, forearm, hand and fingers

When It Is Bundled

Under NCCI policy, a compression wrap applied to the same area as a debridement or skin substitute application is generally considered part of that procedure and is not paid separately. When compression is the only service to that leg, or is applied to a different anatomic site, it can be reported on its own with documentation.

Compression As Documented Standard Care

Most skin substitute policies require documented compression for venous leg ulcers before advanced therapy. The note should show the type of compression, the dates it was used and the patient's adherence. This documentation matters even when the wrap itself is not separately paid.

Compression Supplies

Medicare covers certain gradient compression stockings and wraps for patients with an open venous stasis ulcer under the surgical dressing benefit, and since 2024 covers lymphedema compression treatment items under a separate benefit created by the Lymphedema Treatment Act. These supplies follow DME MAC rules, not the application codes above.

Frequently Asked Questions

Can 29581 be billed with 97597 on the same leg?
Generally no. The wrap is bundled into the debridement on the same area. Check the current NCCI edit table for the specific pair and any modifier allowance.
Does an E/M visit change whether compression is payable?
A separately identifiable E/M visit is reported with modifier 25. The compression application stands on its own documentation either way.
